How Much Does In-Home Care Cost in Central and Northern Virginia?
One on One Care charges $31 per hour for personal care and companion care, $36 per hour for specialized care, and $425 per day for single-person live-in care. These are our actual rates, and they are the same in every community we serve — we do not quote one number in Fredericksburg and another in Culpeper.
Most home care agencies will not tell you this until you have sat through a sales visit. We think that is a waste of your time. If the number does not work for your family, you should be able to find that out in thirty seconds, from your kitchen table, at eleven o'clock at night when you are actually doing this research.
Rates current as of August 2026 and subject to change. Current clients always receive advance written notice before any rate change takes effect.

Companion care
Companion care is help with the parts of daily life that have gotten harder — meals, laundry, light housekeeping, errands, reminders, and the simple presence of someone who notices when something is off.
Personal care
Personal care is hands-on assistance: bathing, dressing, grooming, toileting, transfers, and mobility. Same rate as companion care. We do not charge you more because your mother's needs changed.
Specialized care
Specialized care is care that requires advanced caregiver training — dementia and Alzheimer's care, Hoyer lift transfers, two-person assists, hospice support, and clients with complex behavioral needs. If you are not sure whether your situation falls here, ask us during the assessment. We will tell you before care begins, not after the first invoice.
Live-in care
Live-in care places a caregiver in the home for a full day at a flat daily rate. For someone who needs presence around the clock rather than continuous hands-on care, this is dramatically less expensive than hourly — roughly $12,900 a month, compared with about $22,500 a month for the same coverage billed hourly.

Monthly figures use 4.33 weeks per month.
For comparison, a semi-private room in a Virginia nursing home runs roughly $10,250 a month, and assisted living in Virginia averages about $6,900 a month. For many families, part-time care at home costs a fraction of either.
How Our Rates Compare
Agency rates for personal care in Virginia generally run between $24 and $38 an hour, with the highest rates concentrated in the Northern Virginia and D.C. suburban market. Statewide benchmarks commonly cited for 2026 land around $32 to $35 an hour.
At $31, we sit below the state benchmark — and we want to be clear about why, because a lower price should come with an explanation.
We are not a franchise. One on One Care has been family-owned since 1998, when Charles Murray founded it. There is no parent corporation taking a percentage of every hour you are billed, no national marketing assessment, no franchise royalty built into your rate. What you pay goes to the caregiver in your home and the local staff who supervise, schedule, and back them up.
That is the whole difference. It is not thinner staffing, it is not cheaper caregivers, and it is not a lower level of oversight.
What's Included in the Rate
- ✓A free in-home assessment before you commit to anything
- ✓RN oversight. A registered nurse develops your plan of care and conducts supervisory visits every 90 days
- ✓Caregiver background checks, credentialing, and ongoing training
- ✓Backup coverage. If your caregiver is sick, finding a replacement is our problem, not yours.
- ✓Scheduling, payroll, taxes, and workers' compensation — all carried by us, not you
- ✓A local office you can call and reach a person who knows your family's situation
Minimums, Extras, and the Fine Print
Minimum shift: 4 hours. Minimum per week: 12 hours.
Holidays are billed at time and a half — $46.50 an hour for companion and personal care, $54.00 an hour for specialized care. The holidays we observe are New Year's Day, Easter, Memorial Day, Independence Day, Labor Day, Thanksgiving, Christmas Eve, and Christmas Day.
Mileage is $0.56 per mile when a caregiver transports a client in the caregiver's own vehicle. There is no charge for the caregiver's travel to and from your home.
Cancellations: We ask for 24 hours' notice when you can give it. There is no cancellation fee.
No contract. Care is pay-as-you-go. You are not locked into a term, and you can increase, reduce, or stop service without a penalty. Families going through a hospital discharge or a recovery period often need care for six weeks and then not at all — that should not cost you anything extra.
No assessment fee, no deposit, no setup fee.
How Families Pay for Care
Private pay
Most of our clients pay directly. We bill weekly and accept ACH bank transfer and credit card.
Long-term care insurance
We work with long-term care insurance however your policy and your family prefer — we can bill the insurer directly, or you can pay us and submit for reimbursement. We will provide whatever documentation, visit notes, and invoicing format your carrier requires. If you are not sure what your parent's policy covers, bring it to the assessment and we will read it with you.
Medicaid — the CCC+ Waiver
Virginia's Commonwealth Coordinated Care Plus (CCC+) Waiver covers personal care at home for people who qualify. We are an approved provider and we serve CCC+ waiver clients throughout our service area.
If you are approved for the waiver, the hourly rates on this page do not apply to you. Waiver services are paid by Medicaid.
The one exception is patient pay. Some families are assigned a patient pay amount by their local Department of Social Services, based on the client's income. That amount is determined entirely by Social Services — we have no role in setting it or appealing it. Our only role is collecting it. If you have been assigned a patient pay, that is the only money you will owe for waiver services.

Veterans and surviving spouses
VA Community Care. We are not yet able to bill the VA directly. The VA's community care provider network in our region is at capacity, and we are on the waiting list to be added. We would very much like to serve more veterans this way, and we will update this page when our status changes.
VA Aid and Attendance. If you receive Aid and Attendance, you can use it toward our services today. Aid and Attendance is a monthly benefit paid directly to the veteran or surviving spouse, so there is no VA billing involved — you pay us as a private client at the rates on this page, using those funds.
Many families who qualify never apply, often because they assume it is only for service-connected disability. It is not. If you are a wartime veteran or the surviving spouse of one, and you need help with bathing, dressing, or other daily activities, it is worth looking into. Your local Veterans Service Officer can help you apply at no cost, and we are glad to point you to the right office.
Either way, if you are a veteran or a surviving spouse, call us. Even if we are not the right fit today, we would rather spend a few minutes helping you understand your options than leave you sorting through it alone.
Medicare
We do not bill Medicare. Medicare covers short-term skilled home health care ordered by a physician, which is a different service than the ongoing personal care we provide. If Medicare-covered home health is what your family needs, tell us and we will point you toward an agency that provides it.
